Members will see a substantial savings by ordering replacement contact lenses from this service.

Live Outlook Vision customer service representatives (rather than recordings) answer questions and will direct ASP members to Outlook Vision providers.

 

1. To access the vision network: Simply CLICK HERE or call 1-800- 658-1022.  The customer service representative will request your name, group number, membership number and zip code.  Once your information is confirmed, you will be provided with the name and telephone numbers of participating vision locations in your community.  Please make sure to identify yourself as and Outlook Vision member when making an appointment with a participating provider.

 

2. Mail order program: Call 1-800-CONTACTS and identify yourself as an Outlook Vision member.  You will receive an additional 10% off the already discounted service.

 

 Always show your ID card each time you visit the provider.
