provider networks.
How to Best Use The Network & Generate Savings for You and Your Family
1. FIND A PROVIDER:
Appointments must be made with a providers in the network. To locate a
provider please CLICK HERE , or contact a physician services representative at 1-800-236-3609 for the name
of a contracted provider in your area.
2. MAKE AN APPOINTMENT:
When you contact the provider's office to make an appointment, please let them
know your program utilizes an approved provider network and you will provide further
information at the time of your appointment with the Doctor Letter. If the provider would like to know which network they are affiliated with
prior to the appointment, please have them call Provider Relations at 1-800-730-9730.
3. BRING YOUR ID CARD AND DEAR PROVIDER LETTER:
Members must always present their identification card and the Doctor Letter. Present the letter to the receptionist when you arrive at the participating
physician's
office. If the provider's office has any questions, have them call Provider Relations
at 1-800-730-9730.
The participating provider can choose one of three billing options:
Payment Method 1: The physician's office calls the automated repricing system
at 1-800-648-2583 and obtains a repriced contracted amount. You MUST pay the
contracted
amount at that time.
Payment Method 2: The providers may choose to file a claim form rather than use
the automated system. In that instance, the member will receive a letter with
a statement explaining the contracted amount. The member MUST pay the contracted
amount to the provider within 30 days.
Payment Method 3: Some providers may choose to charge 80% of their reasonable
and customary charge and then submit the paperwork. In this case you pay the
80% at the time of your visit. After the bill is adjudicated, if the provider
has overcharged, the provider will reimburse the difference to you.
Please Remember:
This is NOT an insurance plan and there will be no reimbursement to you or to
the participating physician. You are fully responsible for paying the provider
directly for services rendered.
You will have to pay your bill at the time of service in order to get the preferred
rate. A few providers may choose to send you a bill after your receive services.
It is completely up to the provider which method of billing is chosen, payment
at the time of service or billing you after rendering services.
You must adhere to the provider's preference regarding the timing of payment
to
ensure that you receive the preferred rate. If your provider requests payment
at the time services are rendered and you do not pay as required, your provider
may charge you the full standard rate instead of the preferred rate.
Example:
A member has a $120 office visit with a participating provider. The provider
calls
the 1-888-648-2583 number, which faxes back an explanation of savings to the provider.
The contracted rate is $80.00. The member then pays the $80.00 at the point of
service.
By using the in-network provider, the member saved $40.00 or 33%.

As a reminder, we have a special provider service number, 1-800-730-9730, that
your
providers can use if they have questions about the program.
DOCTOR NOT IN THE NETWORK?
If there is a particular provider you would like to utilize, however, the provider
is not currently in the network, please complete a copy of the enclosed Nomination Form and submit it to the address listed on the bottom of the form. The provider
will be contacted with
regard to network participation.
