Terms & Conditions of Participation
Renewal Conditions: By joining a plan, you are authorizing The AccessOne Program (AP) to bill your
credit card or checking account for the plan you have selected. The charge shall
remain in force until you notify AP, in writing, of its cancellation. By joining,
you are agreeing to the terms and conditions of the plan and adopting it for a
minimum of one year. The plan will automatically renew at the end of your membership
term on an annual basis, and your credit card or bank account will be automatically
charged or drafted for the appropriate amount.
Termination Conditions: AP reserves the right to terminate plan members from the plan for any reason,
including non-payment. Terminated members will receive a refund of all unused
portions of their membership.
Cancellation Conditions: You have 30 days from the day you receive your member card and/or materials to
use the plan risk-free. If for some reason, within 30 days you are dissatisfied
with the plan and wish to cancel and obtain a refund of any membership fees paid
less the non-refundable enrollment fee, please send a letter of cancellation with
a request for refund and your name; member number and group number to:

You may cancel your membership at any time. This cancellation must be in writing

Any unused portions of your fee will be returned at that time less the non-refundable
enrollment fee.
Limitations Exclusions, Exceptions: This program is a discount program and NOT an insurance program. No portion of any provider fees will be reimbursed or otherwise paid by AP,
its affiliates or any voluntary membership association of which you may be a member
as a result of your participation in the program. AP is not a licensed insurer,
provider of medical services or items to individuals. You will receive discounts
for medical services from providers who have contracted to do so with the plan.
You are obligated to pay for services at the time of your appointment. Savings
will be based on the provider’s normal fees. Actual savings will vary depending
upon location and specific services or products purchased. Verify such services
with each individual provider. The discounts under this plan are not available
in conjunction with any other discount plan or program of insurance. The discounts
contained herein and the prices quoted are current, however, subject to change
without notice. Any procedure performed by a non-participating provider will not
be discounted. From time to time, certain providers may offer products or services
to the public at prices lower than the discounted prices available through this
program. In such event, members will be charged the lowest price. Discounts on
professional services are not available where prohibited by law and may not be
available on all procedures. Providers are subject to change without notice and
services may vary in some states. It is the member's responsibility to verify
that the provider participates in the plan. AP, at any time, may eliminate a provider
or provider network from the plan and substitute another provider or provider
network(s) at its sole discretion. AP cannot guarantee the continued participation
of any provider; if he or she leaves the plan you will have to select another
participating provider. Providers contracted by the plan, the networks or AP are solely responsible for
the professional advice and treatment rendered to members and AP, its affiliates
and the voluntary membership association disclaim any liability with respect to
such matters. Services and service providers may change or be discontinued at any time without
notice.
Complaint Procedure: If you would like to file a complaint or grievance regarding your plan membership,
you must submit your grievance, in writing to

AccessOne provides savings to its members on medical services through a number
of medical networks. In order to access these networks and the related discounts,
member or member’s dependents must pay the medical providers promptly. Payments
on all medical bills are due and payable at the time of service. The member has
no out-of-network benefits and must use a contracted provider in order to receive
any benefits.
As a service to members, the AccessOne medical savings program may provide network
rate information to medical providers under this program. If the information
provided results in an underpayment to a medical provider, member agrees to pay
the medical provider for any shortages within ten (10) days of notice to such
member of the inappropriate reimbursement. If the information provided results
in an overpayment to a medical provider, AccessOne will assist member, to the
best of its ability, in collecting any such amount from the appropriate party.
Neither AccessOne nor any of its affiliates, nor any network accessed shall be
liable for any payment to a provider accessed under the AccessOne program, or
any refusal of participating providers to accept the network rates offered under
this program. AccessOne, its affiliates or any network accessed, is not an insurer,
guarantor or underwriter of the responsibility or liability of Member for Member’s
or Member’s dependent’s medical care or any other goods or services provided to
Member or Member’s dependents.
The providers listed in this directory are subject to change without notice.
Member may call the AccessOne provider referral line at 800-236-3609 for current
provider information.
Participating medical providers are independent contractors, and AccessOne and
its affiliates and its contracted networks are not responsible for health care
provided or the omission of the provision of health care by any provider. AccessOne
does not practice medicine or in any manner interfere with or participate in the
provider-patient relationship. All health care decisions are between the patient
and a provider. The selection of a provider is the obligation and decision of
the patient and is not based upon the credentialing or any recommendation by AccessOne,
its affiliates or its contracted networks.
DISCLOSURE:
1. This plan is NOT a health insurance policy.
2. This plan offers discounts on certain healthcare procedures at participating
providers
3. This plan does not make payments directly to providers of medical services
4. The plan member is obligated to pay for all healthcare services but will receive
a discount from those healthcare providers/networks that have contracted with
the discount medical plan organization.
